POSITION OVERVIEW: Reports to the Program Director. Responsible for implementation of the CIS Model at the assigned school.Build and Maintain Effective Relationships: Develop relationships with school staff, parents, volunteers, and partners; provide orientation/training; complete annual Needs Assessment and School Support Plan; launch School Support Team; organize family engagement and student activities; collaborate to support student success; communicate effectively (e.g., Restorative Practices).Coordinate Needed Services & Provide Case Management: Design/implement/partner to coordinate programs, services, and resources benefiting prioritized students and the whole school (Tier I/II/III); broker resources; provide case management in academics, basic needs, behavior, CCR, physical/mental health, and social/life skills; monitor ABCs and SEL; advocate for access; referrals; maintain secure case files.Collect, Report, and Analyze Data: Enter data and complete required reports (monthly/quarterly/annual) in CISDM; adjust plans using data; evaluate effectiveness; create midyear/end-of-year reports.Implementation: Support programming in new schools (Needs Assessments and Support Plans); provide initial Tier I supports.WORKING CONDITIONS: Full-time, in-person presence in assigned school and other district sites as needed; reliable transportation; consistent attendance; ability to meet deadlines.SALARY AND BENEFITS: Mileage reimbursement (IRS rate); $30/month cell phone reimbursement; employee health insurance 100% covered and access to dependent coverage; access to vision, dental, life; employer-paid STD and access to LTD; 401k match up to 3.5%; school holidays and July off; one sick day/month.
